Therapetics service dogs are trained to work with individuals with physical disabilities. They perform a variety of tasks to increase one's independence, assist with daily tasks, and reduce reliance on other people.

The application and approval process takes an investment of time as there are several steps to complete.

               

Request an application package by contacting the office (918-270-4226) or by filling out the form below.

Complete your application package and return it to Therapetics.

All applicants who meet our basic requirements are interviewed by our
Client Selection Committee.  This is a group of dedicated health care
professionals with a passion for improving the lives of those with
disabilities by facilitating their partnership with a service dog.

Following the interview the Client Selection Committee will
make a final determination.

If an applicant is approved for a service dog they are then partnered
with the appropriate dog as soon as possible.

Applicants that receive a service dog must participate in team training
at our facility in Tulsa, Ok. This training requires several weeks.
Please note:

Therapetics specifically trains service dogs for individuals with physical disabilities.

We don't train Guide Dogs, Hearing Dogs, Diabetic or Seizure Alert Dogs, or dogs for individuals with Autism.
