APPLY FOR A SERVICE DOG
Therapetics service dogs are trained to perform a variety of tasks to increase one's independence, assist with daily tasks, and reduce reliance on other people. Please note, Therapetics only trains and places Service Dogs for veterans and first responders diagnosed with PTSD and individuals living with physical disabilities that limit their mobility. Our service area is limited to the state of Oklahoma. We are able to provide referral information for other types of service dogs as well as for individuals living out of state.
APPLICATION PROCESS
The application and approval process takes an investment of time as there are several steps to complete.
-
All applicants must complete the full application, including medical and personal recommendations, in order to move on to the next stage of the application process and be considered for a service dog.
-
All applicants who meet our basic requirements are interviewed by our Client Selection Committee to better learn how a service dog may benefit their life.
-
Following this preliminary meeting, the Board of Directors will evaluate the application and determine if we are able to provide a dog to meet the applicant's needs.
-
If an applicant is approved for a service dog, they are then partnered with the appropriate Service Dog in Training when they become available. Please note that our current wait time is between 2-3 years or more.
Applicants that receive a service dog must participate in 100 hours of team training in Tulsa, OK. After team training is complete and the pairing is considered successful, the handler and service dog are required to complete a Public Access Test before being considered an official service dog team.
Note: You will have to create a free Jotform account to complete an application. The application is lengthy, but you can save your progress and return to it to complete at any time. Your application will not be reviewed by Therapetics until it is fully submitted.
